Parent Survey Preschool Special Education
Dear Parent,
Special Education is intended to help families of children ages three to five get the services and supports they need. We have been asked by the Michigan Department of Education to help find out families' views about Special Education.
Would you help us by filling out this questionnaire? This is your chance to tell us your thoughts and feelings about Special Education and about the services your family has received. Your responses will help guide efforts to more successfully involve families in their children's education across Michigan.
The questionnaire is voluntary and confidential. This means that you do not have to fill out the questionnaire, but we will ensure your privacy if you do. We never report the answers of individual families to anyone. All of the information we collect will be reported as group results. If you prefer, we can help you complete the questionnaire over the phone if you call us at 1-888-800-8865.
Please note: To complete the online questionnaire, type in the login ID and password. Enter only the numbers from the survey code on the top right-hand corner of the questionnaire as the login ID. Enter the entire survey code without the dash into the password field. Type the letters in the survey code as capitalized letters.
By completing the questionnaire you are agreeing to participate in this study.
